WHAT SHOULD A PATIENT BRING TO THE FIRST VISIT?

Regardless of what reason you are coming to our office, certain items should be brought with you to make your visit as productive as possible. First, a patient should always carry a paper or card in their wallet or purse with the following information A) medication list with names and dosages B) list of allergies to foods and medications Note: if you have no allergies to anything, then write none, don't leave this blank C) list of surgeries D) list of medical conditions E) current insurance card F) name, phone number, and address to your primary physician. A medication list must be updated everytime a change occurs with addition of medications or deletions. Also, update the dosages if adjustments have occurred. Listing allergies and the reaction you have to the allergen.

WELCOME TO OUR OFFICE

     This office is in Palm Beach County, centrally located on Okeechobee Blvd.  We provide all medical needs for the lower extremity to our patients.  We perform general examinations, routine care, wound care, surgery, and diabetic treatment.   We have a DME service( durable medical equipment), which provides wound supplies, shoes to those that qualify, and ancillary products to dispense upon necessity for specific ailments.  We encourage you to call our office to confirm that we accept your insurance.  On behalf of everyone in our office, we look forward to seeing you in the near future.  Check back at our site regularly because we intend to provide general Podiatric updates.  Thank you.
